This is confirmed by Washington state death records and her Social Security application form. The death record gives her last name as "Leavens" and indicates a DOB of 21 June 1883. Daisy Bell Knowlton mar. Edward Samuel Johnson (1877-1920) in Snohomish, WA on 28 Dec. 1912 Daisy's third marriage was to Robert J. Leavens in King Co., WA on 7 Nov. 1941."
